Account Options Form
Use this form to make changes to existing accounts only. Changes to account options include: Address Change; Telephone Access; Online Account Access; Distribution Options; the Automatic Investment Plan (AIP); the Systematic Withdrawal Plan (SWP); and Bank Information.
Additional Investment Form
Use this form to make a subsequent deposit into any account type.
Use this form if you wish to update the Address of Record or the mailing address for your account. You may also call (800)860-3863 and press “0” to make an address change.
Automatic Investment Plan (AIP)*
The Automatic Investment Plan (AIP) allows you to automatically have funds transferred from your bank checking or savings account into your Muhlenkamp Fund account on a regular basis. Use the Account Options Form to establish or amend an AIP for an existing Muhlenkamp Fund account.
Bank Information Update
Use the Account Options Form to update or establish bank information used for your account.
Certification of Trust Form
This form is provided as a substitute for submitting your entire trust document.
Combined Statement Enrollment Form
Muhlenkamp Fund quarterly statements have been combined for all accounts that a common Social Security number and address. This has been done to provide you with a convenient overview of your investments on one report. Please use this form if you would rather receive a separate statement for each of your accounts. This enrollment form may also be used if you wish to link accounts registered in different names and Social Security numbers to produce one statement for quarterly mailings. Please that linking accounts does not change the authorization required for account transactions. Mailings other than quarterly statements will not be consolidated.
Cost Basis Method Election Form
Use this form to make the initial Cost Basis Method election for your non-retirement account or to change a previously elected method. To help you understand the different Cost Basis Methods, please refer to this Cost Basis Reference Guide or consult your tax adviser to determine which Cost Basis Method best suits your specific situation.
Distribution Options Change
Use the Account Options Form to update your preference on how distribution and distributions are made in your non-IRA account.
Letter of Instruction Form
Use this form to provide instructions regarding your account. If you have any questions please call (800)860-3863 and press “0.”
Limited Power of Attorney Form
Use this form to establish a Limited Power of Attorney (LPOA). If you have any questions please call (800)860-3863 and press “0.”
Name Change Form
Use this form if you are the current Account Owner, Joint Owner, Custodian, or Trustee and you have changed your name and wish to update it on the account registration.
Online Account Access
Fund shares may be purchased and sold by making requests through the Internet. You can establish a user ID and password by going to the Account Access page of the Muhlenkamp Fund site. Get there by selecting “Our Mutual Fund” from www.muhlenkamp.com and then clicking on “Account Access” (CLICK HERE to go directly to Account Access). Use the Account Options Form to enroll or update bank information for Online Account Access for your account. Your purchase or redemptions order will be priced at the next net value calculated after your order is accepted by the Fund.
Registration Change Form
Use this form if you have an existing Muhlenkamp Fund account and wish to: 1) register a name change, 2) reregister account from Individual or Joint Account to Trust or Entity Account, 3) remove or add an account owner or trustee, 4) gift shares to an individual or entity, 5) relinquish UTMA/UGMA account to former minor, and/or 6) change custodian on UTMA/UGMA due to resignation of current custodian.
Systematic Withdrawal Plan (SWP) Form
The SWP allows you to initiate withdrawals from your Muhlenkamp Fund account at regular, periodic times and to have the proceeds automatically deposited into your bank checking or savings account or mailed each period by check. Use the Account Options Form to establish or amend a SWP on an existing Muhlenkamp Fund non-IRA account with a minimum balance of $5,000. ( – To establish a SWP on an IRA account, complete an IRA/Qualified Plan Distribution Request Form or a Required Minimum Distribution Form, if requesting a RMD.)

Use this form to request a move of Muhlenkamp Fund shares (CUSIP # 56166Y438) via Transfer-in-Kind (TIK) from an account at another financial institution (OUTSIDE financial institution) to an account held directly with the Muhlenkamp Fund (DIRECT account = held at U.S. Bancorp Fund Services, LLC).
Transfer on Death (TOD) Designation Form
Use this form if you have an existing account registered as an Individual, Joint Tenants with Right of Survivorship (JT TEN), or Tenants by the Entireties (TEN ENT) and wish to add the Transfer on Death (TOD) designation, or wish to update the beneficiary information on an existing TOD registered account. The account number, Social Security number, and all account options will remain the same. If you do NOT already have an account and wish to open one with a TOD registration, use a Regular Account Application which provides an area for this.
